Between Reality, Emotion, Voices, and Delusions: A documentary review

Schizophrenia is a chronic mental disorder characterized by severe alterations in perception, thought, emotion, and behavior, affecting approximately 1% of the world's population. Its study takes on special relevance within the framework of advances in psychophysiology, as it integrates the exploration of underlying neurobiological processes with their clinical manifestations. A comprehensive review of the scientific evidence on schizophrenia and affective disorders was conducted, with the objective of analyzing and investigating their clinical manifestations, cognitive impairment, therapeutic adherence, and diagnostic evaluation. The study encompassed multiple comparative investigations; these speak to alterations in time perception, linguistic characteristics, medical comorbidities, and the efficacy of pharmacological and non-pharmacological interventions. Within the methodology, we integrated the analysis of neurocognitive studies, clinical evaluations, and diagnostic comparisons between ICD-11, ICD-10, and DSM-5. A theoretical review of qualitative data, medical records, and tests was used as the methodology. The results showed that patients with COS (Childhood-Onset Schizophrenia) present greater impairment in IQ, memory, and executive functions than those with AOS (Adult-Onset Schizophrenia). Alterations in time perception were identified, including that lexical-grammatical anomalies were observed in the language of patients with schizophrenia, as well as a high prevalence of non-adherence to treatment. The ICD-11, which has a more clinical and global focus, with emphasis on utility in lower-resource settings, introduced significant changes in diagnostic criteria compared to its predecessors, and likewise introduced clinical specifiers. It was concluded that schizophrenia and its affective disorders are multifactorial, with a marked neurocognitive and functional impact, including at uncommon ages and with much more difficult processes. The need for personalized approaches was highlighted, integrating genetic, neurobiological, and environmental evidence to optimize diagnosis and treatment.
